A HARMONY GROUP HOME provides a variety of care residential services in a warm and secure group home living environment. Our highly skilled staff helps developmentally and/or intellectually challenged individuals by providing them with guidance and support in various community living scenarios.

Every service we provide focuses on enhancing the capabilities and quality of life of each resident, helping them achieve optimal advancement in activities of daily living.

The services we provide include:

• Skilled nursing care – An on-site licensed practical nurse (LPN) is available 24/7 to ensure that care plans are being followed and to help administer medications.
• Adult residential services – We help meet the needs of individuals who seek community placement in group homes with adequate staff, appropriate equipment, and quality amenities. These homes allow optimal growth and development while ensuring their safety, improving their skills, exposing them to recreation, and encouraging them to socialize.
• Community living – The care and treatments plans that we follow are customized according to each resident’s needs, preferences, and interests. These personalized plans are enhanced by case management and patient advocacy.
• Individual support services – We provide a plethora of support services to residents with chronic medical conditions and developmental or intellectual disabilities. These include outreach assistance, 24/7 in-home supervision, encouragement of social interaction, and nurse monitoring.
• Community-supported living arrangements – We provide a plethora of support services to adults with chronic medical conditions or developmental/intellectual disabilities including outreach assistance, 24/7 in-home assistance, socialization encouragement, and nurse monitoring.
• Individual and family support services – We provide support services that are specifically designed to cater to the needs of individuals with developmental disabilities, as well as their caregivers and families. We offer patient advocacy, education, resources, and short-term respite care.
• Behavioral support – We focus on delivering resources and treatments plans to address psychiatric challenges. We continually monitor and develop referral management plans, therapies, clinical consultations, and 24-hour crisis support systems with the help of interdisciplinary team members.
• Transportation services – We help residents get to their medical appointments, social events, and recreational activities such as shopping. Through quality transportation services, we help accommodate and enhance community integration.
• Recreational activities – Recreational activities can benefit the overall wellness of each resident. This is why we provide them access to family visits, outings, entertainment venues, religious services, physical health and wellness events, and more.
